What the gateway claim actually says

These are very different claims with very different policy implications, and they get discussed as though they were one claim.

The evidence that supports a gateway effect

It is not thin, and it should not be waved away.

Multiple longitudinal cohorts, following the same adolescents over several years, have found that those who vaped at the start were significantly more likely to have tried cigarettes by the end. The US National Academies of Sciences, Engineering, and Medicine reviewed this literature in 2018 and concluded there was substantial evidence that e-cigarette use increases the risk of ever trying conventional cigarettes among youth and young adults, and moderate evidence for increases in the frequency and intensity of later smoking.

There is also a plausible mechanism, which matters. Nicotine dependence is not product-specific. Once a brain expects regular nicotine, any delivery device will satisfy it, including one that comes in a paper tube. A dead vape battery at a party and a friend with cigarettes is not a hypothetical scenario.

The evidence that complicates it

Population trends point the other way. If vaping were recruiting large numbers of new smokers, youth smoking should have stalled or risen. Instead, in both the US and the UK it dropped to record lows across the same period. Something is either diverting would-be smokers or replacing them.

Common liability is a serious alternative explanation. Adolescents who try any substance early tend to score higher on sensation-seeking and impulsivity, and are more likely to have smokers in the household and in their friendship group. Those traits predict vaping and predict smoking independently. Studies that adjust carefully for them see the association shrink, though usually not vanish.

Direction of travel is often reversed. A large share of adult vapers were smokers first. Every study restricted to never-smokers has to work with a small, unusual subgroup, which makes the effect harder to measure and easier to overstate.

Trying is not taking up. Much of the gateway research measures “ever tried a cigarette”, which for a teenager can mean one puff at a party. Whether that becomes daily smoking is a separate question, and the record-low smoking rates suggest that for most, it does not.

Where the honest middle sits

Probably here: there is a real gateway effect for some individuals, and at population level it is currently being outweighed by vaping displacing smoking among people who would have smoked anyway.

Both statements can be true because they describe different levels. A specific 15-year-old who takes up a mango disposable genuinely may be at higher risk of trying cigarettes than their non-vaping classmate. And the national picture can still improve, because the same product is pulling would-be smokers away from tobacco faster than it is pushing new ones toward it.

Neither of those is a reason to be relaxed about a teenager vaping, which brings us to the part that gets lost.

The gateway nobody argues about

While the debate runs, something less speculative is happening: adolescents are becoming dependent on nicotine.

That is not a risk factor for a future harm. It is the harm. Nicotine affects the developing brain, and the concern about adolescent exposure is well established (Truth Initiative). A young person taking three hundred puffs a day from a 50 mg/ml disposable has a dependence problem now, regardless of whether a cigarette ever enters the story.

The adult version of the same thing is the switch that never finishes. Someone leaves cigarettes for a vape as a harm-reduction step, which is a genuinely good move, and then stays there for six years because the second half of the plan was never given a date. The endpoint quietly moved from “quit nicotine” to “quit tobacco”, and nobody announced the change.
