Heated tobacco marketing says smoke-free and reduced chemicals. What each claim actually means, and the point where independent evidence stops agreeing.

Two claims do most of the work in heated tobacco marketing: that the product is smoke-free, and that it exposes users to substantially lower levels of harmful chemicals than cigarettes.

Both are defensible statements about specific measurements. Neither is a statement about whether the product will make you ill, and that gap is where almost all of the confusion lives. This piece takes each claim seriously and then says exactly where the independent evidence stops going along with it.

“Smoke-free”

What it means literally. Nothing burns. The stick is heated to roughly 350°C rather than the 600 to 900°C at the tip of a lit cigarette, so what comes off is an aerosol, not smoke. No ash, no glowing ember, no side-stream emission between puffs. That is genuinely different, and the term is not a lie.

Where it drifts. “Smoke-free” is a phrase the public already knows from smoke-free legislation, where it means an environment protected from tobacco smoke because tobacco smoke causes disease. Borrowing that phrase moves a chemistry fact into a health frame without ever making a health claim.

Independent position. The World Health Organization treats heated tobacco products as tobacco products, subject to the same Framework Convention obligations as cigarettes, and advises that they be included in smoke-free policies. Air-quality studies detect nicotine, fine particles and volatile organic compounds in rooms where heated tobacco is used. Less than a smoky room. More than an empty one.

“Reduced levels of harmful chemicals”

What it means literally. The manufacturer’s figures describe an average reduction in the measured levels of a selected group of harmful and potentially harmful constituents, compared with the smoke of a reference cigarette, under laboratory conditions. Independent laboratories broadly agree with the direction: several signature combustion toxicants are substantially lower.

Where it drifts. A reduction in measured chemicals is a hypothesis about health, not a finding about it. Independent analyses have also found some compounds present at levels comparable to or higher than in cigarette smoke, which is what you would expect from a different process rather than a milder version of the same one. And independent re-analyses of the manufacturer’s own clinical trial data, published in peer-reviewed journals, have argued that the differences in biological exposure markers between heated tobacco users and smokers were smaller and less consistent than the headline framing suggests.

The regulatory line. This one is unusually clean. In 2020 the US Food and Drug Administration authorised IQOS to be marketed with a reduced exposure claim, and explicitly declined to authorise a reduced risk claim, because the evidence did not support one. That is the single most useful sentence in this entire debate, because it was written by the body that read all the submitted data.

“A better choice than continuing to smoke”

What it means literally. For an adult who smokes and switches completely, ending combustion plausibly reduces exposure. That is the strongest version of the argument and it is not unreasonable.

Where it drifts. The word carrying the claim is completely, and complete switching is the minority outcome. Surveys in the markets where heated tobacco launched earliest consistently find that most users also still smoke cigarettes. For a dual user, combustion never ended and total nicotine intake usually went up. The benefit described in the claim requires a behaviour most users do not perform.

“Designed for adult smokers”

What it means literally. The stated target market is adults who would otherwise continue smoking, and marketing materials say so.

Where it drifts. Heated tobacco is frequently sold in ways cigarettes cannot be: dedicated boutiques, lifestyle photography, device colourways, launch events, and in some markets promotional channels closed to cigarette brands. Tobacco-control researchers and the WHO have raised this repeatedly, and surveys in several countries report use among young people with no prior smoking history. Intent stated in a press release and reach observed in the population are separate measurements.

The history that makes everyone cautious

This is not the first product introduced as a lower-harm cigarette on the strength of better measurements.

Filters, from the 1950s, were marketed on the basis that they trapped particulates. Machine-measured yields fell. Population health did not improve as promised.

Light and low-tar cigarettes, from the 1970s, measured lower on standard machine tests because of ventilation holes in the paper. Smokers unconsciously compensated by inhaling more deeply and covering the holes, and decades of data found no reduction in disease. Regulators in many countries eventually banned the descriptors outright.

Both times, the chemistry was accurate and the health conclusion drawn from it was wrong, because human behaviour sat between the measurement and the outcome. That history is precisely why the WHO and the FDA now insist on the distinction between exposure and risk, and why “we measured less of this compound” is treated as the beginning of an argument rather than the end of one.

What a fair summary looks like

  • Nothing burns. True, and a real difference.
  • Several harmful chemicals are present at lower levels. Broadly supported, with exceptions.
  • Users are exposed to less of certain toxicants. Authorised by the FDA as a claim.
  • Users face lower risk of disease. Not established, and specifically refused as a claim.
  • Bystanders are unaffected. Not established.
  • It is a route out of smoking. Only if switching is complete, and usually it is not.

None of this requires believing anyone acted in bad faith. It only requires noticing which claims are about laboratories and which are about people.

Marketing describes a product. It does not describe your habit, and your habit is the thing with the numbers attached.

So, two minutes: count today’s sticks. Not an average, not an estimate, today’s actual count. Then write down what a pack costs you and multiply by 365. Whatever the chemistry turns out to say in twenty years, those two numbers are already true, and they are the only part of this you control.
