Is IQOS Safer Than Cigarettes? What the Evidence Says

What changes when tobacco is heated instead of burned
The tip of a lit cigarette burns at 600 to 900°C. That fire is what turns dried leaf into roughly 7,000 chemicals, at least 70 of them known carcinogens (CDC), plus tar and carbon monoxide. Almost all the disease burden of smoking traces back to combustion rather than to nicotine itself.
Heated tobacco holds the stick at around 350°C. Below the fire threshold, you get nicotine and an aerosol without sustained burning. Independent laboratory analyses generally confirm the direction of travel: levels of several signature combustion toxicants are substantially lower than in cigarette smoke.
The same analyses add two caveats that rarely make it into advertising. Some thermal breakdown still happens at 350°C, so the aerosol is not clean air. And a handful of compounds have been measured at levels comparable to or higher than in cigarette smoke, which is what you would expect from a different process rather than a gentler version of the same one.
Reduced exposure is not reduced risk
This is the sentence to hold on to, and it is not a rhetorical trick. It is the exact line drawn by the regulator that examined the product most closely.
In 2020 the US Food and Drug Administration issued modified risk orders for IQOS. It permitted the manufacturer to say that the system reduces exposure to certain harmful chemicals. It explicitly did not permit a claim that the product reduces harm or the risk of tobacco-related disease, because the evidence submitted did not support one.
The World Health Organization goes further. Its position is that heated tobacco products are tobacco products, that all tobacco use is harmful, and that reduced levels of some toxicants have not been demonstrated to produce reduced disease. Under the Framework Convention on Tobacco Control, heated tobacco is covered by the same obligations as cigarettes.
Why the caution? Because chemistry is a proxy, not an outcome. Filtered and “light” cigarettes also measured lower on standard machine tests, and decades of data showed no benefit to the people smoking them. Measuring less of a thing in a laboratory is a hypothesis about health, not a finding.

Read that table twice. The top half looks like an improvement. The bottom half is where your next ten years actually get decided.
What switching does and does not buy you
If you smoke a pack a day and switch completely, some things change quickly. Carbon monoxide falls. The morning cough often eases within weeks. Your clothes, car and hands stop announcing the habit. Those are real and worth having.
Three things almost never change.
Your dependence. Nicotine arrives on a similar curve, so the cravings keep the same schedule. Nobody switches to heated tobacco and finds themselves wanting it less.
Your spending. Sticks are priced close to cigarettes in most markets, and there is a device on top. A pack a day is 365 packs a year either way.
Your quit date. This is the expensive one. Switching feels like progress, and feeling like progress is exactly what removes the urgency to finish. People who intended to switch for six months are frequently still on it four years later, having never had a single day free of nicotine.
The question worth asking instead
“Is it safer?” is unanswerable right now, and it is also the wrong question, because almost nobody is choosing between cigarettes and heated tobacco from a standing start. You already smoke, or you already switched.
The answerable question is: is this a step in a sequence, or a destination? A step down that ends at zero is a good decision. A step down that becomes permanent is how the tobacco industry keeps a customer for twenty more years while everyone involved feels reasonable about it.
Most people who stop for good did not manage it the first time. Several attempts is the norm, not a sign that you are bad at this. What separates the attempt that sticks is usually not willpower, it is having a real number to work from.
