  • Fine and ultrafine particles. Aerosol droplets small enough to reach deep into the lungs. Counts rise measurably during use and fall again quite quickly afterwards.
  • Volatile organic compounds and carbonyls, including acetaldehyde and formaldehyde at low levels, plus glycerol from the carrier.

What is largely absent is the combustion signature: side-stream smoke from a smouldering tip, tar, and the high carbon monoxide levels that make a smoky room a smoky room. There is also no continuous emission between puffs the way a burning cigarette emits constantly whether or not anyone is inhaling. That is a genuine difference and it is why the levels are lower.

Lower is not the same as safe

The evidence gap here runs in a specific direction, and it is worth being precise about it.

Second-hand cigarette smoke is one of the best-established environmental health hazards there is: decades of population data, causal links to lung cancer, heart disease, childhood asthma and sudden infant death. That body of evidence exists because cigarettes have been used indoors, at scale, for a century.

Heated tobacco reached market around 2014. Nobody has thirty years of bystander exposure data, so there is no equivalent finding in either direction. “No demonstrated harm to bystanders” and “demonstrated to be harmless to bystanders” are different sentences, and only the first one is true.

The World Health Organization’s guidance is to treat heated tobacco products as tobacco products and to include them in smoke-free policies. Many countries have done so; others have not, and the rules change quickly. Legality varies. The precautionary logic does not.

The people it matters most for

If you are weighing this up, the honest way to do it is to think about who is in the room rather than about averages.

Children. They breathe faster relative to body size, they spend time on floors and soft furnishings where residue settles, and their airways are still developing. This is the group where every second-hand exposure question resolves the same way.

Pregnancy. Nicotine crosses the placenta and affects fetal growth and heart rate. Second-hand exposure is a lower dose than smoking, not a zero one.

Anyone with asthma or a respiratory condition. Fine particulate matter is an irritant regardless of source, and people with reactive airways often report symptoms in rooms where heated tobacco is used, even when they cannot see anything.

People who did not choose it. Housemates, partners, colleagues, taxi drivers. The absence of visible smoke removes the social signal that would normally prompt someone to object, which means consent quietly stops being asked for.

The residue question

There is a second-order version of this that gets less attention. Nicotine deposited on surfaces does not simply vanish; it settles on walls, upholstery, car interiors and clothing, and can react with other compounds in the air over time. That process is well documented for cigarettes, where it is called third-hand smoke.

For heated tobacco the deposits are lighter, but nicotine is still leaving the aerosol and landing somewhere. A car used daily for two years is not chemically identical to one that was not. The practical implication is small and easy: ventilation while using, and not treating the car as a booth.

Practical, non-preachy rules

If you use heated tobacco and share space with people:

  1. Ask, rather than assuming. The lack of smoke means nobody will tell you it bothers them until it has bothered them for months.
  2. Not in the car with passengers, especially children. Small sealed volume, high concentration, no escape.
  3. Open a window and use the same window every time. Concentrating the residue in one place is better than distributing it.
  4. Keep bedrooms out of it. Eight hours of low-level exposure a night is not a trivial fraction of anyone’s day.
  5. Do not use “it’s not smoke” as the whole argument. It is technically accurate and it is not an answer to the question you were asked.

What this is actually about

Most people who ask whether heated tobacco harms others are not asking a toxicology question. They are asking whether they can stop feeling uneasy about using it at home. The honest answer is that the unease is proportionate: less than cigarettes, more than nothing, and unmeasured over the long term.
