The CDC’s list of what has been detected in that aerosol includes nicotine, ultrafine particles small enough to reach deep into the lungs, volatile organic compounds, cancer-causing chemicals and heavy metals such as nickel, tin and lead.
None of that means it equals cigarette smoke. It does mean the honest answer to “is it harmful to people around me” is not zero.
How secondhand vape differs from secondhand smoke
The difference is genuine and worth understanding properly, because it explains both why exposure is lower and why it is not absent.
A lit cigarette is a continuous emission source. It burns between puffs, and that sidestream smoke — the plume rising from the tip — is the majority of what fills a room. A vape emits nothing between puffs. The only thing that reaches the air is what the user exhales, after their own lungs have retained a large share of it.
So the arithmetic is different by design. Fewer emissions, no combustion, no tar, no carbon monoxide, far lower particle mass in the room.
What survives is the part people do not expect: nicotine, and very small particles. Studies measuring non-users who live with vapers find measurable nicotine absorption, at levels lower than in homes with smokers but higher than in homes with neither. WHO’s position follows from that — the aerosol is not harmless and non-users should be protected from it.
Who this actually matters for
For a healthy adult who occasionally shares a room with someone vaping, the exposure is small and the honest framing is “low risk, not no risk”.
Four situations move it up the list:
Babies and young children. They breathe faster relative to body size, their lungs are still developing, and they put hands and objects in their mouths after touching surfaces. Nicotine is also acutely dangerous if swallowed, which makes an accessible bottle of e-liquid a far bigger hazard than the aerosol ever is.
Pregnancy. Nicotine affects fetal brain and lung development. Reducing a pregnant partner’s exposure is a straightforward thing you can do this week.
Asthma and other airway conditions. Ultrafine particles and propylene glycol are airway irritants. People with asthma often report tightness or coughing in rooms where someone has been vaping, and they are not imagining it.
Cars. This is the one people underestimate most. A car is a very small, poorly ventilated box, and exhaled aerosol reaches concentrations there that it never reaches in a room. Cracking a window does surprisingly little.
“I only vape when they are asleep”
This deserves a straight answer rather than a lecture, because it is the most common arrangement in households with children and it is a genuine attempt at doing the right thing.
It removes the direct exposure, which is the biggest part. It does not remove the residue. Propylene glycol, glycerol and nicotine deposit on surfaces — sofas, screens, car interiors, the arm of the chair you sit in — and stay there. It is nothing like the tar film cigarettes leave on ceilings, but nicotine has been measured on surfaces in homes where people vape indoors, and the crawling, touching, mouthing age group is exactly the one that meets surfaces most.
It also does not remove the thing that turns out to matter most in the long run.
The exposure that is not chemical
Children of parents who use nicotine are more likely to use it themselves. That is one of the most consistent findings in the entire tobacco literature, and it does not require a single molecule to cross the room.
What travels is the modelling: that this is what an adult does when stressed, bored, waiting, celebrating or thinking. A device that appears in someone’s hand fifty times a day teaches a watching child what the normal response to a feeling looks like, long before they are old enough to have an opinion about it.
That is not said to make anyone feel worse. It is said because it is the reason many people finally quit — not the aerosol, but the audience.
What to do if you live with people and vape
Practical, in order of how much they change:
- Take it outside. Not a window, not the balcony door, outside. Everything else is a partial measure.
- Never in the car, especially with anyone else in it. This is the single highest-concentration setting there is.
- Lock up liquids and devices. Swallowed e-liquid is a poisoning risk in small children and the strengths sold today are high.
- Ventilate properly if it has happened indoors. Cross-ventilation, not a cracked window.
- Wipe the surfaces you actually touch. The arm of the chair, the desk, the steering wheel.
- Count what you are taking. Every puff you do not take is exposure that never happens, to you and to everyone else.
That last one is the only measure that improves on its own over time. The others hold exposure steady; reducing intake is the one that trends toward zero.
